Where it is, as of August 2026
Novo Nordisk's Wegovy pill (oral semaglutide 25mg): FDA approved 22 December 2025, US launch 5 January 2026. UAE 3 June 2026, the first market outside the US. MHRA approved 11 June 2026, UK on sale 6 July 2026. European Commission authorisation EU-wide on 15 July 2026, with Germany the first EU launch in September 2026 and no other country date announced.
Eli Lilly's orforglipron (Foundayo): FDA approved 1 April 2026; MHRA approved 10 August 2026, the first approval in Europe; no EU approval yet.
Switzerland is outside both systems and has neither, Swissmedic was still processing the applications as of mid-2026.
A contradiction worth logging: one research pass concluded from the EMA EPAR pages that neither oral product is EU-approved. Five independent passes found the 15 July 2026 European Commission decision with a primary Novo Nordisk source. The EC decision is the better-evidenced position, but the EPAR reading is a reminder to check the register before relying on it in a specific market.
What it deletes
The injectable pen requires 2–8°C, must not freeze, tolerates 30°C for at most 48 hours in transit, and must be discarded six weeks after opening regardless. UK operators ship it in insulated wool pouches with seasonally-adjusted ice packs, one in winter, two in summer, plus a separator so the drug does not freeze against the pack, by next-day courier only.
The tablet stores at 20–25°C and must not be refrigerated. A plastic bottle in a padded envelope, by ordinary tracked mail, letterbox-sized.
That removes the packaging cost, the courier constraint, the weekend gap, the temperature-excursion risk and the weather-driven wastage. It also removes the operational moat: cold-chain competence was one of the few real advantages a serious pharmacy had over a newcomer.
It is growing the market, not splitting it
80% of new Wegovy-pill prescriptions in the US went to patients who had never taken a GLP-1. Five million US prescriptions by mid-July 2026 at roughly 267,000 a week, about 90% of the oral obesity category.
The UK proved a European market absorbs it in weeks: roughly 300,000 patients started within three weeks of the 6 July launch, and Wegovy's UK share went from about 30% to 45%.
And the price is going down
UK pill pricing starts around £99 and tops out near £189–£219, against £169–£339 for injectable Mounjaro. Revenue per patient-month falls.
It keeps falling. Novo announced a 50% cut to the US Wegovy list price effective January 2027. France's negotiated reimbursement price of €195.10/month feeds external reference pricing across much of Europe. And orforglipron is a small molecule: roughly 170–310kg of input per kg of active ingredient, against about 13,000kg/kg for peptide synthesis. That 40–80x difference in material intensity is why Lilly built a $6.5bn plant in Houston and why orforglipron can be sold profitably at prices semaglutide cannot reach.
The counter-signal: UK list prices went the other way in September 2025, when Lilly raised Mounjaro by up to 170% to close the gap to the European average. Model both directions.
The adherence problem is different, not smaller
One tablet, first thing, empty stomach, no more than 120ml of plain water, then thirty minutes before any food, drink or other medication. Swallow whole.
Discontinuation from side effects is about the same as the injection, 6.9% versus 6.8%, but the failure mode is a daily habit rather than a weekly one. If retention is where your differentiation lives, the product just changed the shape of the problem.
