After raising UK Mounjaro list prices by up to 170% on 1 September 2025, and amid reported patient stockpiling, with some ordering seven or eight pens at once across multiple providers, Eli Lilly suspended sales to UK wholesalers, causing shortages at Boots, LloydsPharmacy, Superdrug and Well.

In its place came LillyDirect, a manufacturer-run home-delivery route for private Mounjaro prescriptions, initially through exactly two named dispensing partners: Pharmacy2U and Blueco Healthcare. On 3 August 2026 eligibility opened to GPhC- and PSNI-regulated pharmacies running private weight-management services, with Lilly citing protection against counterfeit product entering the supply chain.

Application process, allocation and pricing terms are not public. For anyone building in the UK, that is the single most valuable phone call available.

LillyDirect launched in the UK in June 2026. Critically, UK law prevents manufacturers from shipping prescription-only medicines direct to patients, so it operates through independent prescribing and dispensing partners, with Lilly stating it plays no role in clinical decisions. That legal structure preserves a role for the intermediary layer, for now.

No equivalent EU-wide manufacturer channel was confirmed. Novo runs NovoCare Pharmacy in the US and partners with Ro, LifeMD and Hims; LifeMD stated on its Q2 2026 call that it receives no economics from either manufacturer, which is a fair summary of the intermediary's bargaining position today.

Injectable pens need 2–8°C end to end, insulated packaging, seasonally-adjusted ice packs and next-day-only couriers. One secondary source reports MHRA approval for Wegovy to travel at up to 30°C for a maximum of 48 hours during delivery, which would materially lower the barrier, flagged as single-sourced, verify against the SmPC before designing packaging around it.

The oral pill removes the question entirely: 20–25°C, do not refrigerate, padded envelope.

MHRA seized nearly £45m of illegal medicines in 2025 including over 5,680 GLP-1 pens, and called an October 2025 Northampton raid the largest single seizure of trafficked weight-loss medicines recorded worldwide. Ireland's HPRA saw GLP-1 seizures rise from 1,582 to 48,752 packages in a year. France's ANSM has referred cases to prosecutors and is lab-testing counterfeit "patches". Malta arrested a doctor and a gym instructor over a peptide black market in August 2026.

The relevance to a legitimate operator is twofold: it is why manufacturers are locking down distribution, and it is why the compliant story is worth telling to the trade.
