How it works here
Ownership [1][2][3]
The Pharmacy Act 2007 permits a pharmacy owner to be a sole registered pharmacist, a partnership, or a corporate body. Each pharmacy goes on the PSI Register of Retail Pharmacy Businesses, first-registration fee €4,297 under S.I. No. 171 of 2026. Where a corporate body owns several pharmacies, only one superintendent pharmacist (minimum three years post-registration) is required for the whole group.
Separately, s.63 bars doctors from holding a beneficial interest in a pharmacy, defined to include being a director or holding more than 0.5%, and vice versa. A purely non-medical corporate owner is unaffected. A non-EEA founder needs either an EEA-resident director or a Section 137 bond (about €99 for two years).
The legal shape of the market [1]
HPRA states plainly that "it is illegal to sell prescription medicines online in Ireland." There is no PSI registration pathway for online sale of POMs, the Internet Supply List covers non-prescription medicines only.
What every Irish operator actually does is different and legal: an online consultation with an Irish-registered doctor produces a prescription, which a PSI-registered pharmacy dispenses and couriers. That is not "online pharmacy retail". A new entrant should be precise about which of the two it is building.
Counterfeits and the enforcement turn [1][2][3]
HPRA detained 763,027 units of illegal medicines in 2025. GLP-1-type seizures rose from 1,582 in 2024 to 48,752 in 2025: a roughly thirty-fold increase. In October 2025 it issued a health warning after counterfeit tirzepatide pens were found to contain insulin.
On 6 May 2026 it published a notice, "Promoting medicines to the public on social media", explicitly naming semaglutide and tirzepatide and stating that advertising law applies to anyone advertising a medicine in Ireland, social media included. No named enforcement action against a specific operator has surfaced yet. Expect that to change.
Reimbursement, pricing and the pill [1][2][3]
The HSE confirms Wegovy and tirzepatide are not covered by the medical card, LTI or Drugs Payment schemes for weight management. A December 2025 parliamentary answer records them as "undergoing assessment"; an NCPE cost-effectiveness review of Mounjaro was reported in April 2026 with patients meanwhile paying around €600/month.
Consumer pricing splits into a consultation fee (€20–40) plus separately dispensed drug cost, Everwell markets Mounjaro "from €215/month", myBMI lists an own-brand oral semaglutide at €125–149/month.
The oral Wegovy pill is covered by the EU centralised authorisation of 15 July 2026, but as of early August 2026 was reported approved-but-not-yet-obtainable in Ireland. Orforglipron has no EU approval.

Routes in, and walls
What works viable
- Incorporate an Irish limited company, engage a superintendent pharmacist with 3+ years, and register on the PSI Register, €4,297.
- Contract Irish-registered doctors for the consultation layer. This is the exact template every incumbent uses.
- Use Ireland as the English-language EU entity while the UK sits outside the single market.
What does not blocked
- A pure e-commerce storefront listing and selling POMs without a consultation and a PSI-registered dispensing pharmacy. Squarely illegal.
- Any doctor with a beneficial interest in the pharmacy, s.63.
- Assuming advertising latitude because no operator has been named yet. HPRA opened the file in May 2026.
How to make it work
The recommended entry path for a non-pharmacist operator running an own brand with a partner pharmacy. Read it against the six axes above, not instead of them.
Incorporate an Irish company, register with the PSI, and use Ireland as the English-language EU base.
Ireland is the cleanest ownership regime in the EU that also speaks English, and it is the exact template the incumbents use: Zava routes its continental prescribing through an Irish entity. With the UK outside the single market, Ireland is the natural EU counterpart to a British operation.
